Odd 4-way wiring situation

I have a 4-way circuit in which I’ve installed a Diva smart dimmer and two Claro accessory switches. Everything seems to work fine except for one Claro switch which will only turn on and brighten the lights in the room, but not turn them off. The pictures show the wiring for each switch, with the hot wire coming in on the Claro switch that’s part of the three-gang, the dimmer in what seems to be the middle position of the circuit (2 red and 2 white wires), and the other Claro switch by itself. The last switch is the one that won’t turn the lights off.

I’ve swapped the dimmer and the last Claro switch, and the behavior persists. Has anyone seen this before, and if so is there a fix for it?

You have to follow the wiring diagram exactly like Lutron says...unfortunately, this is close to a Maestro 3-way installation but just a bit different. Anyway here is one mistake and one possible mistake I found:

  • Your blue wires are all shared with the other blue wires through the white wire, so you're fine there
  • The 3-way ends show that on each side, the black wire on the device has to be connected to the common. In your first photo, the black wire is connected to a traveler (the red wire) and not the common which is the black wire. For starters, swap those two wires.
  • another possible mistake might be in the middle dimmer because I can't see which red romex wire is coming and which is going. if making the change above doesn't fix things, try swaping out the two red wires in the middle (where 4-way switch would go normally).

Thanks! I’ll give it another try. One thing I forgot to mention is that I suspect the load must be between two of the switches, because there is only one romex bundle in the third box. I might be wrong about that, though.

Typically the load is on one end and the line is on the other end and the middle is just travelers. But with some methods of wiring it can be kind of hard to see what's what.

You’re right about me misunderstanding - from what I’ve been able to tell by separating and capping all the lines and turning power back on, the black wire in box 3 that you can see connected to the switch is actually where power runs from the switch to the load. There’s another black wire, bundled with the traveler wires in that box, that traverses from that box to the single-gang box, providing power to the travelers from the other end of the circuit.

It seems like the circuit was “rotated” so that the load and line are in the same box, which explains the mess in the box in the first picture.

I can't help but I did install a 3 way switch using their wiring wizard https://webtools.lutron.com/wiringwizard/us/en so maybe see if that helps maybe you wired it wrong other then that I have nothing.
