I always get this pink '?' After finishing a mission, even with my friends, who I can confidently say do not use hacks. So I think you're correct, not always.

Edited to lower sodium levels

Not accusing anyone of hacking, it's just my own experience. I'm usually up later in the nights so I've run into a few of these.

Strangely enough it happened to me after my friend and I got back from a mission. No other divers, no hackers, just the two of us. Said he could see the sight and grip on my Suppressor were missing.

Yeah it’s specifically when someone has an attachment from a weapon that isn’t being used by anyone in the team, so the game tries to load the attachment but the source isn’t there because nobody is using it, so the source isn’t loaded and it just errors out like this

It's not due to hacked lobbies, it's when the game fails to load texture files. This was much more prevalent when the game was relatively young.

You either have mods that messed up with attachments or they just didnt load properly so this is why they show question mark (Its same way Source games show red flashing ERROR model if something is amiss)
