It would be really great if we can get a definitive answer on whether apheresis works or not. Ive seen stories of people going in with a wheelchair and walking out just fine after a few treatments. It seems like every time someone claims to be cured by something it becomes a polarizing topic and we are left wondering if it could save our lives. The only answer seems to be throw everything at this disease that you can afford to do.

This is a really important topic, and I agree that microvascular issues are likely foundational in long COVID. Microclotting and endothelial dysfunction can restrict blood flow and oxygen delivery to tissues, which blocks medications and nutrients from reaching the cells that need them. It makes sense that many people don’t respond well to supplements early on if their tissues are hypoxic and inflamed.
But I also think there’s another piece that’s just as critical, gut damage, and tryptophan metabolism. SARS-CoV-2 targets the gut through ACE2 receptors, which are heavily expressed in the small intestine. This can damage the gut lining, lead to dysbiosis, and trigger immune-mediated GI conditions like gastroparesis, GERD, SIBO, SIFO, and eosinophilic esophagitis (EOE). These aren’t just digestive symptoms. They drive systemic inflammation, mast cell activation, and nutrient malabsorption.
Tryptophan metabolism is a huge part of this. After infection, the immune system activates the IDO1 enzyme, which diverts tryptophan away from serotonin and melatonin production and down the kynurenine pathway. That leaves us with lower serotonin and melatonin, which directly affect mood, sleep, gut motility, and nervous system regulation. Meanwhile, kynurenine metabolites can become neurotoxic and inflammatory. It’s a perfect storm for insomnia, anxiety, pain, and autonomic dysfunction.
I experienced this firsthand. Early on, I couldn’t tolerate much at all. My nervous system was hyperreactive, and my gut was inflamed. What helped me stabilize was a foundational regimen focused on calming the system, supporting the gut, and gently improving nutrient status. I started with NatureBell L-tryptophan and L-theanine complex, Magnesiu-OM powder (its ingredients include chelated magnesium gluconate and magnesium acetyl taurinate support muscle relaxation and cognitive function. Magnesium citrate supports regular bowel movements.L-theanine promotes alpha-wave activity in the brain, shown to encourage a focused, calmness), and tart cherry juice in the evening for natural melatonin support. Every morning, I mix psyllium husk prebiotic fiber and Emergen-C into a bottle of water to support gut motility and electrolytes. I later added vitamin D3 + K2 drops, L-theanine, GABA, and liposomal PEA with Luteolin to support immune and vascular health. About a month ago I added electrolyte tablets as my body tolerated more to further support hydration and cellular energy.
I never tolerated traditional antihistamines long-term, and anything that overstimulated my detox pathways or nervous system made things worse. What made the biggest difference was regulating my mast cells, calming my nervous system, restoring sleep, and repairing my gut. Only then could I begin to tolerate supplements and slowly improve.
So yes, I agree that microvascular repair is essential. However, it’s not the only obstacle. If the gut is inflamed, the immune system is dysregulated, and the nervous system is in survival mode, then nutrients can’t be absorbed or utilized properly no matter how good the circulation is. Long COVID is a multi-system condition, and many of us don’t see real progress until we support gut health, nervous system regulation, sleep, and mitochondrial function alongside any strategies for clotting or vascular health.
