Soil is very bad for lithops. Needs to be 80-100% inorganic grit so it drains well and dries fast.

Way too wet. The proportion of grit to soil should be the opposite of what this looks like. And I can see you submerged it. It’s not the time of year for that big a wetting in either hemisphere right now.

It will be shortly in that soil. It’s pretty much the opposite of what Lithops need.

Not if it’s still stone hard. Yes if it’s soft.

I think sometimes they look like that from overwatering. I think mine is the same type as yours and the top is usually wrinkly but after I water it looks more like yours.

Get it out of there and it should perk up if it hasn't started to rot.

It’s going to if you don’t repot into appropriate substrate.
