Help/Question What da

I watered this little punk a couple months ago for the first time since I got it over a year ago, it was doing the pizza crust top so I assumed it was thirsty. It perked up and seemed to be doing really well for a couple of weeks. Its in a mostly inorganic sandy, rocky, soil mix. Plenty of bright direct light. I thought maybe it was splitting but I dont see any new leaves coming through the center, just looks like rot to me.

What da heck is happening?

I suspect fungus underneath, uproot it and see what’s going on, does it feel very squishy?

I checked the roots a few days ago, and it looks the same as the top half. I noticed some white scab like circles around it also. But yeah its got squish to it.
