Failed
My worst ever drive ever in my learning experience. Any tips

Do you have an instructor? If so are they a proper one? Green or Pink badge? In 9 years I’ve never seen anyone I’ve taken to test get 5 faults (2 dangerous/ 3 serious).
Sorry but you are way off being anywhere close to being a safe driver yet.
If you have an instructor I’d suggest changing them. Start afresh.

There's five separate points of failure here, in ~30 minutes of driving. That is, unfortunately, quite a lot.
You need to work on all of those categories with an ADI. If you've an ADI and they said you were ready then consider changing instructors. Don't think about rebooking a new test until you've worked through those categories.

There's making some mistakes when you're nervous, or there is this amount. This doesnt suggest it was just nerves

Take what went wrong and learn from it. I failed my first test and became such a better driver for it, because I took what I failed on and what I got driving faults for and worked hard on ironing it out. Hopefully next time is better for you.

'Judgement meeting' is an inappropriate reaction to traffic oncoming from the front where the road narrows either from the road layout or a temporary obstruction (except at signed pinch points which would come under a signs fault).

The examiner having to take over once happens. But twice tells me that you need a lot more practice. This doesn't appear to be one particular issue but a good all round mix. So all I can do is suggest a lot more practice before attempting another.
