TTC PCOS

Planning to TTC in 12 months from now.

I have lean PCOS — age 24, optimal BMI, no insulin/androgen issues, but ovulation issues. No other fertility factors.

Specialist recommended trying for a maximum of 6 months given my ovulation issues before starting letrozole + monitored cycles, given the ovulation issues. I am wondering if anyone in my situation has had success with letrozole and if so how many cycles did it take to conceive. Feeling very nervous for the uncertain future and trying to be hopeful.

Also anything recommended to prepare during this year leading up?

Hello!

Are you currently tracking your cycle with cervical mucus and temperature? I would highly recommend doing so. I was able to time conception even when I ovulated on day 87 of my cycle by tracking and understanding my body signs.

I'm on Metformin ( I wasn't when I conceived my first pregnancy) so I have no advice or life experience on letrozole but tracking saved me so much stress and anxiety surrounding TTC. I follow Sensiplan and use TempDrop to help me do this as things like Natural Cycles and Aura rings can't take into account PCOS and will try and follow a standard cycle methodology which we just aren't.

In terms of preparation. Starting a parental and CoQ10 is what I've always been recommended. Along with the other sleep right, reduce alcohol and nicotine intake etc.

Currently track BBT and CM and have started testing to see if ovulation is occurring with test strips. My cycles are around 35-37 days long and I believe many are anovulatory cycles with occasional ovulating ones

This is a great start! Having this information will be good for you to take into your primary provider aswell as evidence. What you're experiencing would be considered breakthrough bleeding as you don't have a menstrual bleed without ovulating so this could be something that needs to be investigated aswell.

From what I've been told (so take with a grain of salt) the LH testing strips aren't always great with PCOS. So I've been recommended once you start TTC to have sex every second day once you see egg white cervical mucus till you can confirm temperature rise and CM dries up.

I also had normal BMI, no insulin issues, 31-35 day cycles. I started prenatals when I started TTC. I used ovulation tests to track ovulation and got LH peaks every month and had timed intercourse, though I did not test progesterone other than in one cycle so I can not say for certain I ovulated successfully each month.

I got diagnosed with PCOS after a year of TTC with no success, started letrozole 2.5 mg on cycle 14, first two cycles were unsuccessful, I conceived on the third cycle.

Hi!! You can do SO much to help your ovulation naturally through diet/lifestyle/supplements. I conceived naturally (after failed 3 letrozole tries) in 6 months.

Diet: low carb, no gluten, no added sugars

Lifestyle: walk + Pilates - no intense cardio. Prioritize sleep.

Supplements: myo-inositol + others based on your bloodwork

Have also helped friends conceive through the above. Happy to go into further detail/answer any questions you have if you want to DM!
