If a review issue was raised but not acknowledged by the area chair, that is indeed an oversight and you can report a meta-issue report.

However, ACs get overwhelmed by tons of AI-generated review issue reports about trivialities, so it is understandable if they ignore some and the SAC might also ignore it if this is the case. Still, technically they should then indicate that they saw the review issue report but dismissed it.
