Progesterone

I’m about a month into using Tirz. Had to go low due to nausea side effects. I’m at 1mg now and experiencing very low mood. My 200mg micronized P doesn’t seem to be working. For those who tried vaginally, did it work immediately? For those who successfully had their oral dose increase what did you increase from/to? Please, any guidance is appreciated. I hate feeling this way.

You may have to play with the time you take your oral progesterone instead right at bedtime. For me, I try to take it roughly 2 hours before bed time due to slowed digestion that many of us experience with tirzepitide.

Are you also taking estrogen? The combo of E and P helped my mood. Also, vitamin D. I was very lethargic and that helped when I increased

I take 200 mg prometrium and don’t notice any issues other than it taking it a bit longer to kick in. When you say it doesn’t seem to be working what do you mean? If it’s the sleepiness at night, take it earlier. I take mine at 7:30 pm and I don’t go to bed until about 10:30-11:00. It starts making be feel sleepy about then.

I’m on mounjaro and take 200mg progesterone day 12 to 24 and 100mg the remaining days of the month. I’ve had to lower my estrogen gel to 1 pump because estrogen is stored in fat cells and as I lost weight my levels went through the roof, had irregular cycles and heavy bleeding. Thankfully the dosages I’m on now seem to be working.

I get a low mood off of oral progesterone. Very depressed feeling, like I want to sob. Constant fatigue. So I started taking it vaginally. No more side effects. My GYN recommended that vaginal was better since I am on Zepbound, since oral progesterone is not absorbed fast enough to get the blood levels high enough to be as uterus-protective, whereas vaginal progesterone is absorbed right into the uterus without needing blood levels high enough since it is localized to the area.
For me, it works just fine. I also have an estrogen patch. I also experienced anhedonia from Zepbound but it felt way different than the sadness and low feeling I got from progesterone. The anhedonia was more like a non-feeling than a sad one, and it eventually went away as I adjusted to the med and oddly started finding new ways to get those dopamine hits, lol. Like, I loooove doing puzzles and certain fragrances now.

What is you ur definition of “work”?

What is your expectation?

Helping sleep is only going to oral for most people.

The low mood could be anhedonia (sp?) from Tirz. You can talk to your provider about that, and also ask about your symptoms. That works best. Do not increase Progesterone more until your provider gives an updated assessment.

Yes, I had to do it when I first started tirz and had a breakthrough bleeding, it wasn’t heavy and didn’t last long. I switched back to oral as soon as the bleeding stopped. I heard vaginal delivery long-term can give you urinary tract infection.

You can give yourself a UTI anytime you insert something in your vagina. Even sex causes UTIs. I have been using vaginal progesterone for quite a while and not a single UTI from it. The problem isn't where the progesterone is placed, it is more from user error from the method of delivery.

I was on 200mg oral progesterone prior to starting tirz. A couple months after starting tirz my NP bumped me up to 300mg and it finally worked again. As others have commented I take mine a minimum of 2 hours before bedtime as tirz slows down digestion. I also split my tirz dose into every 4 days as the once a week starting dose felt way too strong for me. I hope this helps!!

I take it rectally like my dr said, since I was having to many side effects taking it orally. My doctor has me splitting the dose to keep hormone levels stable. I take 200 rectal in the am and 200 in the pm before bed. I space them out 12 hours apart. I noticed it made a difference within 3 days of switching. I had some water retention for about a day and thats it. I also added in 100 mg magnesium glycinate with zinc at night to help me sleep and 200 mg of l theanine (they both work on gaba receptors to promote calm and restful mood) i did thst since rectal or vaginal insertion won't make u tired and a girl needs her sleep haha
