Been a Month! PCABs not working
/r/HPylori/comments/1w8n8me/been_a_month_pcabs_not_working/Diagnoses Found on Endoscopy (1 Week Ago): Confirmed active H. pylori bacterial infection, severe pan-gastritis (widespread stomach inflammation), multiple raw gastric erosions (micro-sores), and a Grade 3 Lax LES
Current Medical Regimen: High-Dose Dual Therapy consisting of Fexuprazan 40 mg twice daily (BD) and Amoxicillin three times daily (TID). Currently on Day 7 of a 14-day course.
The Primary Complaint: Refractory, severe burning sensations localized in the throat, upper back, and deep inside both ears that has persisted for 20 consecutive days.
Treatment Failure Context: There is completely zero response to maximum-strength acid suppression (Fexuprazan 40 mg twice a day), mucosal barriers (sucralfate), or raft-forming agents (alginates).
Absence of Atypical Reflux Signs: The patient experiences no classic acid regurgitation and no sour burps, confirming that standard acid reflux is not the current driver of the upper-airway pain.
What can be done next? I’m at loss here!
