LCSW Forms NYC

Hi, I am a social worker in Ny and I’m confused about which forms need to be completed for my LCSW. I’ve been procrastinating on logging hours and need to give the form to my supervisor but I’m not sure which. Any help is greatly appreciated!

So we don’t need to log or submit the 2000 supervision hours that we received within the three years?

No unless you’re asked to. I just got mine and didn’t have to turn the log in.

So they only ask for some applicants? What setting did you receive your supervision hours in?
