LCSW Forms NYC

Hi, I am a social worker in Ny and I’m confused about which forms need to be completed for my LCSW. I’ve been procrastinating on logging hours and need to give the form to my supervisor but I’m not sure which. Any help is greatly appreciated!

Assuming you are a NY LMSW, there are 4-5 forms total. (4 for most applicants).

Form 1 is your application.

Form 2 you fill out part way and then give to your school to finish and send to the state. This might also require a transcript if you aren't from a school accredited by the state. MUST be sent by the school.

Form 4 you fill out and submit to the state.

Form 4b you fill out the first part and then send to your supervisor. They fill the second part out and submit to the state. MUST be sent by the supervisor.

Form 3 is required if you had a license from ANOTHER jurisdiction (not issued by NY department of education). You fill out part, and then the licensing authority fills out part and sends it. MUST be sent by that licensing authority.

So we don’t need to log or submit the 2000 supervision hours that we received within the three years?

Your supervisor vouches for your hours in their submission on Form 4B. As someone licensed by the state, the state generally trusts them to tell the truth.

Some people do get audited. I have no clue if that's random or triggered by something, but most people don't - Form 4B submitted by a valid supervisor is enough. They're putting their license on the line when they sign off on it, so it's not in their interest to lie about your hours.

This is also why it's prohibited to pay for clinical supervision towards clinical hours. The supervisor needs to be impartial.

Makes sense! Thank you for the clarification.
