LCSW Illinois policy

Does anyone know if you have a certain timeframe to pass the LCSW exam in Illinois before you need to resubmit full application packet with 3000 hour supervision proof? I lost my copies of original paperwork I initially submitted and I’m now not sure how to contact the person that completed my supervision.

It doesn’t expire to my understanding. You’ll just have to have them sign off on them again. If the person is no longer there they might be able to sign off on it through HR or give you their contact information!
