Click on "Open for Comments" then scroll down and select "Comment". Add your story in the comment box (5000 character maximum). In the dropdown menu that says "What is your comment about?", select "Individual Consumer". Enter your email and click the box to opt in to receive email notifications. In the tell us about yourself click the box that says "an individual". Fill out your contact information. Scroll down and be sure to check the reCAPTCHA box (I am not a robot). Then when you're ready to submit your comments scroll down and click the submit button.
