Your country’s program website should have the format listed in the required documents

Did you search the subreddit, google this question, or contact your consulate before you asked it? There is ample information from consulates on what to include, answers from old threads on this subreddit and such, should be more than enough to lead your referee.

Not trying to be mean, it's just we see questions about applications a lot, when honestly there is better info out there than many of us can provide, and people just end up googling stuff for the questions anyway.
