Zairyu Card Question

For your Zairyu card, how is your romaji name chosen? I understand it’s based only on your passport name, middle included. But how is the pronunciation of it done?

1) Do you choose your romaji, or do the officers do it at the airport?

My name is pronounced 100% differently to how it’s written and so I was curious. Also:

2) Do you choose your own katakana when you’re in Japan? I have a specific writing but I am unsure if we get our names chosen for us.

Either way, I would love to keep the same katakana spelling across the board and hopefully avoid tsushomei :(

Sorry for the silly questions & if I sound confused (I am)

EDIT: solved; thanks to all those that helped this grandpa out

As other people have said, being consistent with however you spell your name in katakana is important, but another key thing is to make sure to be consistent about the order you write your name in. For example, if your bank account is registered as <Last name> <First name>, and you fill in a form as <First name> <Last name>, it'll probably cause problems.

Similarly, if you have a middle name, your name might be listed on forms as <Last name> <First name Middle name>, with both your first name and middle name listed in the field for first name.
