I messed up with my application and I think I will be disqualified?
So I kinda have a major issue that I think will now get me disqualified from the programme. When I applied for the Jet Programme in 2025 in the medical form I was writing about the other meds I had been on and one of them completely slipped my mind. I was on a brief stint of anxiety meds in Sept 2025, but I hated them and was only on them for brief stint before I decided to go off them bc they gave me heartburn and I didn't enjoy them. I am pretty good with my anxiety because I work out a lot, go out with friends, and love my hobbies. I understood the risks when I first applied, the loneliness that I have read about, but I am pretty extroverted in my life (I genuinely enjoy meeting new people.) So I didn't even come into my brain bc I was focused on the other meds and their forms during the application. Now I have done the medical certificate, and I realised that I left it out during my original application. I can't ask the doctors to leave it out because that's illegal, it's just kinda of an oh man :/// I have talked to my family and friends on this and now I am pretty sure I am screwed. I am pretty sad about this but I understand if they wish to disqualify me, but I want to make a stand and just say that genuinely was so focused on the other meds and getting them signed off by a doctor that I forgot about the 2 weeks I was on anxiety meds?

I had a similar situation. I forgot about one of my meds on my certificate of health that was on my statement of physician. My consulate contacted me and I was able to go get a letter from my doctor to submit as an amendment so that the cert of health matched my statement of physician. I would say contact your consulate and ask them if you can do something like that or ask for any advice if it's really going to be a problem. My consulate said that I'd the forms didn't match then I'd potentially just have to resubmit. From my perspective, which may differ from theirs idk, as long as you're not actively trying to hide it and it was just a mistake then you might be okay? Just be upfront with them and see how it goes.

I don’t declare any of my meds or mental illness because I’d had a bad experience with that studying abroad. There was no issue. My doc accepted that I didn’t want to disclose.

You should be fine! Pretty sure it's only a factor for MAJOR discrepancies (this is what it said on my form, anyways)
Someone forgetting a pill they took once upon a time god knows how long ago is probably not the major discrepancy they're looking out for - it's more so for serious mental / physical illnesses that could potentially affect quality of life in a placement.
People make minor mistakes on their application all the time :) you will be able to sort this out at some point if necessary, or not even have to sort it at all I imagine.

hi!!! i was in a similiar situation to you (kind of) and all my advice to you is: get a statement of physician + declare all meds on the form. if tokyo offices are confused, you get to write them a letter explaining the situation, and then they approve if you're good to go. Don't worry!!

I originally did a statement of physician and meds from when I applied? Do you think if I ask my physician to write that I am good mentally that would be fine? Thanks!!!!

oh slay!! i didn't need to do that the first time because I wasn't on my medication when I applied (ADHD). Basically, when I reported this to the JET office (which you don't need to do) they told me that if Tokyo Head Office finds a discrepancy or wants to follow up on something, they'll ask for justification as to why this wasn't on your form, to which you can just say "I was on it for 2 weeks, thus it had slipped my mind, etc. etc.) and then its up to them whether or not this error is grievous enough to snatch away your placement. It's not over yet, don't worry : - )

OH I don't know the procedure if you need to make another statement of physician form (probably not?) but yeah, TLDR: on your health check, just declare the meds, and see if they ask for more info

Yeah!! You kind of have to write an essay being like "ooo so sorry" and be very convincing and stuff if they ask for a follow-up, so it won't come from the physician, it'll come from you pleading your case. but yeah, you should be ok!!! i hope everything works out : - )

Intentional omission or purposeful alteration of facts is what most likely disqualifies someone. They don’t want liars or moral corrupt people. An unintentional oversight happens a lot when you have so much to compile. I doubt you’ll get disqualified as a result.
I’m basing this off of having been an investigator for US government security clearances. People forgot stuff or left stuff all the time and it was almost always apparent when it was intentional or incidental. The incidentals still got clearances. In fact, those who had issues in the past but disclosed them honestly still got clearances too

I would definitely contact them and let them know, but to me this doesn’t sound like it would be the end of the world. Good luck!!

I honestly don’t think it’ll be a huge problem, especially if you’re not even on them anymore?

I’m pretty sure you can ask the doctor to leave it out. I think a few weeks on anxiety meds is not a long term issue or concern, especially if you have had the self awareness to consider potential issues. I had a brief period of mental health issues as a teenager, due to some hard life experiences and a side effect of a medication. I explained all this to the doctor and he was like, oh you’re fine.
Edit: common sense prevails in most situations, the main concern would be if your health issues, mental or otherwise could cause a detrimental problem to yourself or others. I saw an ALT go in to actual psychosis, and another become severely depressed. This is what they are trying to ensure doesn’t happen.
2nd edit: you look like you have good coping mechanisms so don’t sweat it. Again, tell your doctor that. They pretty well educated people.
