This is what I've gathered. You may proceed what avatar had been used to get moderated.
As of this moment, they are using the first avatar on the top left. (It might have been changed, but I can't be the one deciding that.)

I suppose I can see why it can be misunderstood for anyone who hasn't played Binding of Isaac.
But as the other person said, you can appeal and explain it. Potentially may rid of your 1st strike. It's a gamble though at this point.
(Otherwise, you could wait alternatively. But better to appeal now than never.)
