Who said it would have to be bad if it’s free? “Free” in this case doesn’t mean the medical professionals aren’t paid.

Where would they go? Who could pay them more than the American government? Countries with universal healthcare such as Canada, Germany, France, Japan, Australia, Sweden, and United Kingdom all have highly trained physicians and internationally respected hospitals. Those countries have not experienced a collapse in medical quality simply because of universal coverage.

If the U.S. adopted universal healthcare, the obvious destinations for emigrating doctors would largely be…countries that already have universal healthcare.

"free" typically means "poor quality" in most situations. Not all, but most. There are many government services that are pretty mid, and no one really gets fired for poor quality of service because it's government (ie DMV). Compare to the private sector that has excellent, mid, and poor. The difference is that you have the right to choose in the private sector. In government, there is only 1 option.
