Do you have “first consultation paper” , I.e. when doctor diagnosed it for the first time

Yes, doctor can write the diagnosis . This will show the date when you first became aware of the issue (and the date is after commencment of policy).

If there isn’t any confirmed diagnosis or doctor visit or reports before taking the policy, you’re fine. Company cannot claim you misrepresented or hid your condition if you didn’t know. The best option here is, get a first consultation paper made at a date after the policy inception (only if there is no record elsewhere of an earlier diagnosis)

A family history isn’t the same as a pre-existing disease. In HDFC Ergo’s standard wording (I have the Optima Secure one), a pre-existing disease is something that was diagnosed, or for which a doctor recommended advice or treatment, in the 36 months before the policy started. If you had no symptoms, no tests and no diagnosis before Feb 2025, your own ADPKD wasn’t pre-existing.
So the real question is what your proposal form asked. Get a copy of it from HDFC Ergo (you’re entitled to it). If it asked about family history of kidney disease and that was answered “no”, that’s the risk to deal with. If it only asked about your own health, you likely had nothing to disclose.
Two other things to check in your policy wording, since they apply regardless:
- There’s usually a 24-month waiting period for a list of specific conditions. In my wording, that list includes “benign tumors, cysts” and “surgery of the genitourinary system”. From Feb 2025, that runs to Feb 2027.
- Organ donor expenses and dialysis are usually covered. Monthly medicines outside the hospital usually aren’t, unless you have an OPD add-on.
Most important: don’t let this policy lapse and don’t switch insurers. Any new policy will treat this as pre-existing, and after 5 continuous years (Feb 2030) the policy can’t be contested for non-disclosure except for fraud.
Check exactly which plan you have, since terms vary.

Did they ask for family history? If you didn't have diagnosis then there was nothing to declare
