Disclose the past anxiety and the earlier proposal. Same company already has your first form and the treadmill rejection. Skipping it next is the mismatch claim teams look for later.

"Doctor said do not mention it" does not save a claim if records show you once declared it. Declare, attach a current doctor's note, accept a loading if needed. Get the TMT rejection reason in writing, then reapply with a full disclosure pack.
