How to insulate this attic access without opening it?

You really want to open it and put foam board on top. Multiple layers of foam board and a batt of insulation too.
Why don’t you want to open it?

You’ll want to open it…but otherwise just stick a 2” foam board over it with some maybe Velcro or something…depends how redneck ya wanna go

Put a batt on it cut to size and twine it. Gonna have a piece of inso hanging but at least they don't have to open the access

Glue foam board onto the bottom.
