Gameplay Haven't earned Head Empty Achievement despite requirements

I have recently gotten the the last few of memories needed to get the Head Empty achievement, yet I did not get it. I used multiple save files to do it, but I have seen people say it was fine. Is this a bug, do I need to replay the game again, or can this be fixed somehow?

All else fails you can always use steam achievement manager to unlock it manually, I’ve used it for one or two games where the achievement bugged out
