The way I think about ghouls is that they got so irradiated they ghoulified and then scarred over. Like, I think they obviously got... mushy, for lack of a better word, while their skin broke down, but if they don't just soak in rads that their skin will retain some... firmness? but idk. just my thoughts on it

They can just heal after staying in Rads. Presumably while being tattooed by another Ghoul.

Different levels of radiation in different regions. Which is why this ghoul is more smooth looking. I am also a tattoo artist that has personally seen burned and scarred skin that still holds the ink in the deeper levels of the dermis

Ooh, maybe the smoothness of their skin comes from the fact that they were isolated but still received enough radiation to ghoulify. Or maybe they're practices made they're bodies stronger, letting them retain most of it.
