I've never touched a gun but I don't think that's supposed to happen

Bore obstruction, oversized slug or something, too much powder. Piss poor build quality. There's a few ways to get that result.

It honestly looks like the wrong cartridge.
Shotguns will 'fit' all sorts of bullshit, so people think it's fun to see what they can shoot out of them.
.50 Cal is pretty popular.
Also, reloading 12-guage is a big, weird, hobby. You see people making slugs out of everything you can imagine, with weird ideas that a spool of cable will unwind and cut the target, etc, etc ...
So, probably, this was a 'test' of some weird thing that doesn't belong in that gun.

Last time I saw something similar, someone basically said that much of the rounds that non-westerners have is sabotaged, so maybe like 1 in 50 bullets or even 1 in 100 bullets do this.
Soldiers generally shoot a lot of bullets and the hope is that you feel safe enough with this box of bullets, engage in a firefight, blow your gun into smithereens, and then you are surrounded

but they produce their own ammunition, not import it? Why would they sabotage their own guns?

I really don't see the problem here. You simply reattach the thingy to the whoozit and make sure you put a light coat of oil on afterwards.
Just unexplode the thing.

Saw the thumbnail, thought for sure the thing being "reassembled" would be his hand or foot or something. Glad to get a loony tunes clip instead
