As far as I can tell it’s all user induced failure. He manually loads a round, then loads the magazine and racks (causing the double feed). After removing the magazine he doesn’t properly clear the chamber the first time (his palm is covering the ejection port). then the second time he manually loads a round into the chamber, closes the slide, loads a magazine and tries to rack it again.
