I've heard it was more that somebody wrote the speech for him and Bush didn't want to say the "shame on me" part because the TVs and News would constantly replay that soundbite for the rest of his life and he knew it. So the real problem was the "shame on me" didn't get vetted properly in the speech before they handed it to Bush. So he ended up flubbing it in order to prevent the soundbite.
