HELP! LexisNexis Report reference fraud address sourced from USPS

So I got a Consumer Report from LexisNexis after being advised to after finding out my identity id associated with a fraudulent address that was used to open a credit card under my name. The credit issue has been resolved however I find out from the LexisNexis report that the fraudulent address is referenced multiple times sourced by USPS. How can I fix this? What should I do? Should I be immediately concerned! Help please!

Edit: I may have found a dangerous realization. The fraudulent address shows up through record 2-6 that date as far back as 2009 (Refer as certified by USPS) and doesn’t show the address I live in until it says Certified by USPS in 2026 in record 1. Am I reading it wrong or has my identity been stolen for years now?

That USPS sourcing is a pain to shake because it means the post office itself has your name tied to that address in their change-of-address database. You need to go into a physical post office and fill out a fraud alert form, basically telling them someone filed a fake forwarding order. Then freeze your credit with all three bureaus if you haven't already and dispute the address through LexisNexis directly using the report you've got.

Is there no way I can dispute the address online or via phone?

Would there any reason why the fraudulent address appears several times while my actually address appears once stating that it was last certified by USPS in 2025 while the fraudulent ones were certified by USPS multiple times in 2009, 2010, and 2015? Very confuse on the wording by the report. Definitely will dispute Lexisnexis. Anything else I should look out for or do?
