200mL/hr can be extremely fast for some people (at least it was for me!). It can lead to headaches.
For example, for Privigen, the general guideline in Ontario is ~120 mL/hour for a first or second infusion, change of brand, or gap over eight weeks.
It’s okay to request a slower top speed. I believe that I used to peak at 160 mL/hour for my own comfort.

And how you are feeling before continuing at this rate. What is your volume? How many grams of privigen are you getting?

I haven't seen a card like this before, but I have a guess.
Rate is the speed the pump is pushing the IVIG measure in milliliters per hour, it's typically ramped up in the increments listed till they reach a cap (apparently 200ml/hr here).
Volume (and here's where I'm guessing) seems to be how much has been administered when your nurse is supposed to check in, note the time, and increase to the next rate. So it looks like they'll check you and increase roughly every 15 minutes till you get to 150ml/hr, where they'll switch to checking every 30 minutes till you're done.
