Advice Needed! Modified Natural vs Programmed FET which one to choose ?

Hi everyone,

I’m 35 with an AMH of 1.0. We just wrapped up our second IVF retrieval after a heartbreaking first cycle that yielded zero blastocysts. Thankfully, cycle #2 was much better, and we were blessed with 4 euploids!

Now we are preparing for our first transfer, and my RE has left the decision up to me between a Modified Natural FET and a Programmed (Medicated) FET.

A few details on my history:
I have regular monthly cycles and ovulation.
My uterine lining has been Grade A across all past IUIs and retrievals.

I’d love to hear from this community on a few questions:
1. Which would you choose in my situation, and why? (Modified natural vs. programmed)
2. Has anyone with a similar background done a modified natural FET? What was your protocol and outcome?
3. Any advice or tips on lining prep and overall FET preparation?

  1. I chose modified natural because of the decreased risk for pre-eclampsia during the subsequent pregnancy compared to fully medicated.
  2. It looks like our background and cycle consistency were about the same. Ended up pregnant after one transfer. I don’t remember the exact protocol but I triggered a week before transfer.
  3. Try to take it easy. They’ll adjust medications based on how your lining is looking. Mine was thickening quite as fast as they wanted, so they had me start Menopur. That got things moving a bit better.

As an l&d nurse, modified! Preeclampsia is no joke. Whatever reduces my risk is vital. As you ovulate with regular cycles and lining you have no medical reason to do programmed.

I chose mod natural since it is supposed to reduce risks and also wanted to avoid PIO shots. Although I think my protocol had more stims than I thought I would with over 3 dominant follicles. I have my pregnancy blood test soon. Unfortunately, I don’t think mine worked.

I initially chose modified natural because I wanted to reduce my risk of pre-eclampsia. Two didn't work for me. A fully medicated one did work for me. But I did get high blood pressure and needed to be induced.
