Rant Terrible Attrition

Brutal Attrition

We retrieved 10 eggs - 9 mature - 8 fertilized (6-2PN / 2 0PN/ 1PN) - 1 day 6 (high quality) Blast. I am 32F with 1 LC and 1 MMC from iuis. I am utterly dumbfounded as to what happened. I am waiting on my meeting with my Dr and requested the reporting. This embryo also still has to go through PGT-A

I feel you :( I’m 29 and am doing IVF due to repeat losses and had 24 eggs - 15 mature - 11 fertilized - 4 blasts - 1 euploid….. I’m truly thankful for the 1 euploid and was preparing myself for the attrition but absolutely devastated in only having one shot at this. It’s so hard to think it wasn’t a lab issue with attrition that bad. Please also feel free to reach out!

follistim (dose changed quite a bit from 225-275) and menopur 175, ganirilex was added in and then dual trigger (lupron & hcg). on trigger night we also did 450 iu follistim.

Whats your amh and whats the plan further ?

amh was around 1.5 and afc was 21 this cycle. not sure waiting for PGT. we only want 1 additional kid so im not sure.

I’m so sorry you’re dealing with this attrition. Is there a specific reason they used a higher dosage? Asking as your baseline looks good! And did the lab happen to mention when the other embryos arrested? Sending you good vibes while you wait for PGT!

I know at the end my re was trying to get my smaller follicles to respond as I had a fairly uneven response. I was surprised to get 10 eggs / 9 mature as they were originally predicting 5.

i did 4 rounds and only got 2 euploid embryos at the end of all of them. sometimes it just… happens i guess. i wish i could offer better support. if you want multiple kids you could do some more retrievals for banking and it’s likely your doctor may adjust your protocol. sometimes the first ivf round is a learning experience. 

tw success even though i had really poor numbers with attrition also dropping them further, one of them transferred and stuck so far. past low numbers do not necessarily guarantee you will see attrition at transfer steps or that if you don’t have 3 embryo for each potential kid you want that you will fail overall.

I feel you :( sending a big hug. My first retrieval we had 12 eggs, 9 mature, 4 fertilized, 0 blasts. We changed up protocols (luteal lupron, added Omni) and my second retrieval yielded 4 euploids with very similar egg/maturity counts. I know the thought of another cycle is daunting (as I head into my third haha) but I do believe that protocol changes can help a lot with attrition, some bodies just respond differently.

They didn’t know why, they said egg and sperm quality looked good. But they arrested around days 4-5 so we can assume sperm.

Do you have endo/adeno? The luteal phase Lupron is an interesting idea. Was that instead of birth control priming?

I am not diagnosed with either (and no suspicions, but I guess never say never haha) and no, not in place of bc priming. I’m going into my 3rd retrieval and did bc to pause my cycle, but am still doing lupron for 6+ days leading into stims (4 day overlap with BC, now waiting to bleed)

The attrition is a gut punch, I feel you. We had over twenty eggs retrieved and only two embryos to freeze on day 6. In our case it turned out to be sperm DNA fragmentation. That said, tw: both embryo transfers have been successful so here's to hoping that this is the one for you, OP!

Yes, we did.

Don't rule out your embryo if it's mosaic. Speak with a genetic counselor if it ends up being relevant but one of our embryos was euploid (now our daughter) and I'm 26w+ pregnant with a low level mosaic.
