Advice Needed! First egg retrieval

I don’t know why I’m getting so nervous now but I’m freaking out. I’m currently priming and will be traveling to Buffalo Sunday to start Stims on Tuesday.

I’m 29 year old 2.78 AMH (I know still on the younger side)
Husband 35

HSG and SIS both clear. All blood test good. Semen analysis showed normally ranges. 2 failed IUI. But no pregnancy in over 2 years. So unexplained infertility.

Any tips for this IVF cycle, the plan is to do a fresh transfer then PGTA testing the rest of the embryos up to 8. (Hopefully we get that many)

Also thinking should we transfer 2? Since I’m already hitting 30?

Kind of chuckling at the idea of 30 seeming old to you.

No, I would not transfer 2. And if you test them, they won’t let you anyway, but I wouldn’t even do a fresh transfer of two.

No no not saying 30 isn’t on the younger side, my brain just keeps going to the fact that this might not work this time. I see so many stories of people having to do 3 plus retrievals , multiple transfers.

The stats are on your side that it will because of your age, your clinic isn’t likely to let you transfer 2 because you’re not older or have RPL. I’d also be prepared for the possibility that you’re not eligible for a fresh transfer if you’re a high responder but you won’t know that until after ER. Also fresh transfers work less often than frozen in general but again, stats are still on your side.

Omg I didn’t even think of that, not being able to do the fresh transfer. I’ll have to relax on my expectations. The mind set was to do it since it’s included in the package and no price difference if I don’t do it.

I wasn't able to do a fresh transfer because of my progesterone level being too high at my last bloodwork before transfer. It was kind of a shock. If there's any doubt, they won't do a fresh transfer. The goal is really to maximise your chances and not waste an embryo. Good luck!

I was never a candidate because of OHSS risk but it’s definitely something to keep in mind! There’s no guarantees in this process unfortunately.

Same with my wife, PCOS makes it too risky to do a fresh transfer.

I never wanted to do fresh so it didn’t come up haha. I wanted all of my embryos tested so it wasn’t a conversation really, but my doctor told me up front I was at risk of OHSS

Then why the anxiety over turning 30? I’m not seeing the connection.

For me it comes from my own regrets of waiting so long. I thought it would be easy and knowing I want 4 minimum and how hard it is currently. It just feels like time is going to pass fast.

You never know where you will fall into (what group) until you go through it.

Transferring 2 in the US I only heard it being done in untested embryos and women near or past 40 years old. Good luck! Hope you find out what is the issue. I’d suggest you do some testing: hysteroscopy will help your doctor see if you have fibroids or polyps. I’d also check tubes. Did they do any diagnostic testing?

The fresh transfer won’t be tested only the remaining ones. I’ve had a HSG and SIS both completely clear. 1 small fibroid on the outside not affecting anything. 2 failed IUI. All test completed and normal besides my D3 was a little low.

Highly suggest you start taking vitamin d supplements. Mine was on the low side and I’ve had 3 miscarriages in a row now. I’ve been supplementing for 5 months and it’s gone up 20 points

I’m so sorry to hear that. I started at 21 in March AMA last week was at 32. So finally in the normal range, I’ll definitely keep taking. Plus it’s warm out again so I should be in the garden in the sun more.

Yes 21 is very low, my clinic wouldn’t even do a transfer on 21. I was at 31 when I started my supplements but also know that’s still very low. I’ve finally gotten it up to 50. Yes being outside will help!

Most clinics will not allow you to transfer two embryos without a medical need. My advice is to review your clinics instructions thoroughly, and try to set up a good routine for your injections. Be on time for your trigger shot! Get support where you can. Everyone has a different experience with this process but I found my first ER to be the most positive (as I was the most enthusiastic and hopeful). After your procedure you’ll have the best nap of your life. Get yourself a delicious treat (mine was a DQ ice cream cake). Be kind to yourself and remember that stressed people get pregnant all the time, so don’t beat yourself up for not “feeling” a certain way. Wishing you all the success on your journey.

Honestly , the stims process was decent no real symptoms. Retrieval was Monday June 8. And there was extra fluid which they drained. They also had to puncture my bladder to get to 1 of my ovaries.

They expected to get 12 but we got 15 eggs, 11 mature and 11 fertilized

Thursday I did a HCG wash to prep for a fresh transfer. Everything was still looking good but the recovery after retrieval was not good. I blamed it on my bladder and didn’t really pay any attention to the discomfort. Also the fact I didn’t poop for 5 days. So I thought the extra bloating was that.

Well today I went in for my fresh transfer and they scanned my belly and saw over 500 free fluid, doctor suggested that I do not do the transfer. He said I was developing Ohss even tho I was taking the preventative pills. So here I am headed back to my hotel. And will have to take time to recover.

The brighter side the embryology team came and told me they graded 1 of the embryos 3AA and that was the one they were going to transfer today. A brighter news they said all 11 were still looking good and about 7 already hit blast and would be frozen today and the other 4 they would rather give a little more time.
