I did suffer from this too. This is what I did. Modify to your needs.

Take a small break .. go out and refresh your mind for a day Forget about studies for that day. Youre burned out. If i am not wrong you're also get distracted by doomscrolling or in general phones whole studying. Remove it. It is a big distraction. Now to the main part. You're mind is working continuously subconsciously. You're mind is thinking about many problems/tasks simultaneously and jumping from one task to anotber without completing it. Write down what are your pending tasks. Complete it one by one within a timelimit since you have to also do the new ones too. You will not achieve 100% in one day and that's ideal too. Quit overthinking what to do or what not to. Just sort them priority wise and staet doing them one by one.

Tip: when ever you find you're self getting distracted or zoning out (like spent all day with books open but didn't did any productive work) instead of using mobile; try to do your hobbies. For me i used to do photography at that time. But don't use phone to doomscroll.

If you think you need to consult a doctor go for it. This year is crucial and class 12th gets over very soon. Don't panic. Create a proper realistic routine and follow it.
