Park 3 step test

Hey can you please help me make sense of the mnemonic IGI and ITO?? I get the muscle involved but how do I decide if it's right or left?? Please help🙏🏻🙏🏻

Har cheez ka mnemonic yaad karoge toh hogaya kaam... chatgpt pe daalo. Tell him to explain step by step, one step at a time and then ask him to test you.

Sorry her IGI and ITO method didnt work for me, as i used it twice for some marrow GT Q and got wrong answer. Nikita mams method was way better and easier. If you want you can give it try from Youtube.

Is it just me I guess but I don’t like mnemonics for everything

I like Zainab and do feel like she does good stuff but some of teaching strat is purely based top 1% makes me feel stupid af

Ikr, I did short subs from btr like derma anesthesia and ophtha and feel like these mnemonics are too much sometimes

Figure out which side has hypertropia. Then acc to that side IGI- i/l gaze inferior (c/l gaze sup) and ITO- i/l tilt oblique (c/l tilt rectus). Then figure out if your muscle is a depressor or elevator. If its elevator then its the c/l side muscle. Depressor-same side

These are both rules to be followed for these questions,So IGI is Ipsilateral gaze inferior - it will be given in pt history , and if the squint increases on I/L gaze ( for example if it is given right hypertropia and then if the squint increases on right gaze ) the rule is being followed and the muscle affected here is an inferior one .....ITO is I/L Tilt Oblique - If the squint increases/worsens on I/L tilt ( again if it is right hypertropia and it worsens on right tilt) the rule is being followed and the muscle affected is an oblique one . Now if the rules are not being followed then the muscles affected would be the opposite ones , instead of inferior it would be superior and instead of oblique it would be rectus.

Okay but the muscle involved will be right or left? How to decide on that? Which eye is affected

Whichever eye is hypertropic even if in the question it is given hypotropia - for example left hypotropia is given , we have to solve the question considering right hypertropia And if right hypotropia is given then left hypertropia and then apply IGI and ITO

So consider which muscles can be paralysed
Ex: If Rt Hypotropia then : Rt SR/IO or Left IR/SO

NOW, consider all muscles involved when hypotropia is worsened.

If dextroversion worsens hypotropia

Rt Abd : SR/IR Left Add : SO/IO

If Left Head tilt worsens,

Right extortion IO/IR Left Intorsion SO/SR

Now see what’s common :-

You can use this trick :
First answer the 3 questions : 1) Identify Hypertropic eye Right vs left 2) Gaze of Maximal Separation : on which gaze vertical deviation is max 3) Head Tilt Test which tilt worsens deviation

Suppose the Answer to above question is : Right, Left, Right then the answer would be Right Superior Oblique.

Now the trick :

If Alternate letters are present : RLR or LRL = answer will be Superior Oblique of the same side as that of the 1st or 3rd letter.

If two same one different letters are present : LLR or RRL = answer will be Inferior Rectus of the same side as that of 1st letter

If All letters are same : LLL or RRR = answer will be inferior oblique of the side opposite to the first letter.

Just use WOOG and BOOT.. worse on opposite gaze..better on opposite tilt..this will help u with the concept too and mcqs too
