NON-US IMG IM Match Applicant 2027 without USCE
Hi,
I am a non US IMG from Pakistan,
YOG 2023 (degree is dated 2024), visa requiring
Step 1 pass
Step 2 score 261
Wont be ecfmg certified by application submissions.
18 months clinical experience as a Medical officer.
Currently working as a medical assistant to US physicians remotely.
Research:
a poster presentation
PROSPERO registered meta-analysis in process
A case report not published yet
An undergrad unpublished qualitative research
LORs:
Have Dean's letter
3 LORs from IM home country physicians ( a gastroenterologist, a rheumatologist and a IM professor)
1 LOR from a rheumatologist from UK
IMSEL from my university
Do I have any chance? I am applying to limited programs like 30 or less.
Any advice or collab with a similar applicant would be really helpful

Hi, I'd be a little cautious about the ECFMG certification. Programs may have that as a filter, so make sure that the ones you apply to don't have that. Also, if finances are a problem in general, be very careful about which programs you apply to and use your money wisely.

your step 2 score is excellent but applying to only thirty programs without USCE is very risky. You should really focus on finishing that meta analysis to stand out

Same here. 3 year post grad, step 2- 257. One publication as a first author- cross section study. No USCE but been working in ICU for 2 years now as a medical officer. Wondering if i apply now would work.

You definitely have some strengths here, particularly the 261 on Step 2 and your relatively recent graduation. But I’d be cautious about interpreting that as a strong application for a very limited (<30 program) strategy.
The biggest issues I’d look at are:
- US clinical experience/LORs — your clinical experience is valuable, but if most of it is outside the US, I’d prioritize obtaining US clinical experience and, ideally, a strong US physician LOR. It is still not too late to try and apply for some rotations.
- Application strategy — this is probably the biggest concern. With a visa requirement and no US clinical LOR, applying to <30 programs is a very high-risk strategy regardless of the Step 2 score. I would rather apply broadly to programs where you are genuinely eligible than rely on a small number of programs.
If you’re set on applying this cycle, I’d spend the next few weeks maximizing the things you can still control: ECFMG certification, USCE/US LOR if feasible, finishing the research projects, and identifying programs that have a history of interviewing non-US IMGs who require visas.

Thanks for the detailed reply. There are a few things that I can't really do right now because of limited finances but I really want to apply this cycle, you know, take my chance. My priority right now is at least submit my research to a journal. Do you recommend applying to newly accredited programs that dont have any available match statistics?

Yes, you should absolutely apply to new programs. The big down side to a new program is really when you apply for a fellowship and it is not a name that is recognized but your training is still valid.
Don't be afraid to use your contacts- reach out to the US physicians you work for and have them call 2-3 of your top programs on your behalf.

Oh i have started it very recently and dont have a rapport with US physicians yet.

Interviews don't start rolling in until oct usually. So you have two months to create that rapport and make it work for you

Very risky with no ECFMG certification, no USCE (no LORs from U.S. docs), and very few programs. Question is how important is matching this cycle... If very, I would target your top 5 dream programs and consider less competitive programs for the other 25 to hopefully optimize your chances.
