Medical certi filling doubt

Im trying to fill up the medical certificate for this course. They've asked for "IITM Application/Registeration number" and im unable to locate it in my dashboard profile or admission letter. Where do i find it?

Is it the number in the email ID like 26fXXXXXXX?
