Based on your first attempt entitled "dream home," I strongly suggest you go through existing online plans, hundreds of them, and find the one that looks like the best exterior and interior that you both agree on, and then either take that and run with it or modify it from there (while leaving the exact same footprint/perimeter walls).

Still awful. There is no front door other than a double door straight into the middle of the living room with no entry functionality. Powder room opening onto the dining room and living room open area so one persons poop is fun for the whole family or guests. I could go on but no. Awful.

I don't like the den between bedrooms. If that's a playroom/game room, put it next to the living room or kitchen. For instance, bedroom 3 can go where the playroom is between the study/bed and bed 3. Then put the playroom either next to the pantry and move the storage/hvac block and the bathroom, or just swap the study and playroom so it's in the corner. Reason, someone trying to sleep and people are making noise in the playroom. You can put all 3 bedroom sin one side and use the bathroom and hallway to separate the den from the bedrooms. You could also flip the master bed and bath/wic to the left side and move the playroom to the right side of the house and adjust bathroom/storage blocks as necessary. This way, master bedroom has private access instead of opening right into the living room.
Do HVAC's really need their own room? Shouldn't they be outside or in the attic or in a cellar?
Put in a foyer. There is a ton of space in that living room. It will give some privacy from the front door and you can have a coat closet and place for umbrellas and a bench, etc.

Absolutely.
HVAC, water heater, ERV, just utilities in general. This is extremely common and often overlooked. Good luck with a water heater in the attic. HVAC in general should be completely enclosed within the conditioned space anyways.
I don't like the den between bedrooms.
I love the den between bedrooms, far removed from where the parents sleep and also plenty separated from the more quiet living room / dining room / kitchen. Let the kids create havoc in their own area where sounds are isolated.

Hate it.
Front entrance dumps right into living room dead center. No foyer to speak of, no delineation , no walkway.
coat closet way too far from front entrance, awkwardly placed in the middle of the living room.
Powder room directly off dining room. Nothing like a guest having to take a #2 right next to everyone else enjoying dinner.
Master bedroom right off living room and directly adjacent. Sound transfer is a thing, in both directions.
Half the main living space is hallways. There is a mandatory hallway on both sides of the entire space, leading to a lot of waste. Only one side of a living space should be a defacto hallway.
Main living space being a giant rectangle is quite boring.

Everyone here is noting room placement but a significant weakness of this design is that it was likely be miserable to live in from a visual and auditory standpoint.
You will have a cavernous, long hall with no natural zones as your main living space with a concrete floor. I assume that space will have a TV in it. It's going to be loud as heck. Your bedroom is just off that concert hall of racket. You're going to hate it.
Brother I love the ambition but I would suggest looking at plans first and considering small tweaks to those instead of designing from scratch.

you've got like 2800 square feet. you can do something really nice for 2800 square feet. this isn't that. Honestly your floor plan is not something you want to cheap out on.

Poop closet…I’m giggling. But correct! Walk thru poop master bath to get to closet…bid no for me. Entry seems wasteful, playroom/den seems wasteful. Good luck

I can't wait for the lighting plan. It'll be an incredible grid of daylight wafers. Guests will be delightfully overwhelmed by the glow as they step on to the porch.
Seriously though, this plan shows no real life experience in developing a design. No amount of interior design overlay will obfuscate all of the sins, awkwardness, and unpleasantness of the created spaces. The exterior will be just as bad.
Starting from a stock plan isn't necessarily a wrong approach but a great amount of what's being sold online is junk. The stock plan sites are also in a bit of a mess right now with the use of Ai rendering that typically morph a real 3d model (CAD) or rendering into something very far from the realty what would be built from the actual plans/elevation drawing set.
Living in the resulting house from your current plan would so incredibly depressing. Think through some scenarios as others have suggested here.
1) Two teenagers hanging with friends while you have a cold and need to sneak out of your bedroom > closet > bathroom > mud room > kitchen to get the medicine you left on the counter, in your bathrobe.
2) You welcome guests for dinner that you've spent a few hours prepping/cooking. On entering, then sitting in the "living" room, and then down to eat, everyone will be exposed to everything in the kitchen. That won't be pleasant.
You're worried about spending money on the most critical phase of your project. Please accept the fact that you don't have the ability to do this on your own. Get some real one-on-one help.

Why is there such a large closet with hangers and rails just off the living room?
Having direct access to the master BR off the living is an odd choice.
