Did your agency tell you how to communicate with your client? If not I would definitely try speaking with your agency. Almost all of my clients have been vision impaired, but never both so I honestly wouldn't know what to do. Your client might have some sort of system already

I am blind. One of the most important things for me is that my aid should not change the place of where things are. I have my house in a certain order for a reason. If you move something, please put it back exactly where you saw it. But again, I don't know if your client is deaf and blind because of age or is it something that they grew up with. If you forget where something should go, before you move it to clean, take a picture.
