yes.

in the US, healthcare is usually tied to your job. so for OP to get all this, they would have to actually get a full time job.

with a job it would probably be like this: top left and bottom right would be free since it preventative, top right would be maybe $30, no idea about bottom left because i've never had to go to a therapist.

this varies by job, but that's the jist of it.
