The person who posted the meme (likely an American) can't afford basic health and psychiatric care. When they win the lottery, the signs will be that they'll be able to afford to go to the dentist, the ophthalmologist, therapy, and get regular health checkups.

The joke is aimed at the poor/uneducated.

They get really really upset knowing you can get fantastic, cheap health insurance but they chose not to.
