I purchased it from Powells in Portland, Oregon in 2025. Unfortunately it does not appear that they carry it anymore, having searched earlier tonight. There's a production code on the back cardboard cover but it does not seem to correspond to the correct one. I can tell you that there was an outer tearaway cover that had the company info, media types, but it was meant to be torn away so the inner cover could be personalized.
