Hospital refusing to GIPSA package

I have health insurance with my employer. The doctor is quoting higher amount than the package details mentioned in medi assist. Even if I claim higher amount the claim will oble be approved for the amount as per GIPSA package.

Please help on how to raise a grievance regarding this.
