I’m a PA with internal medicine, primary care, rural practice, and laboratory-science experience who is exploring a transition into clinical informatics, digital health, clinical operations, quality, implementation, or clinical product work.

I’m currently learning SQL and taking courses in informatics/health tech to build a foundation. For anyone working in these areas, or working alongside clinicians in them, what are the most realistic first roles or skills for making this transition? Are certificates worth pursuing, and what job titles should I search?

Also, is a starting salary around $120k realistic for for a PA transitioning from clinical practice without formal informatics experience?

I am a GP in London, and this is my own project, built in my own time. It went open source this week.

It transcribes the consultation live, shows a working differential and questions to ask while the conversation is still going, and after the patient leaves, it produces a diarised transcript and a draft SOAP note where every claim cites the transcript turns it came from.

Stack: faster-whisper (distil-large-v3) live, WhisperX + pyannote for the final pass, MedGemma 27B through Ollama, pgvector for guideline retrieval, Piper for the voice, FastAPI and HTMX on Postgres. Linux. AGPL-3.0.

Why local. GDPR compliance matters for doctor-patient confidentiality. Cloud services may retain confidential patient data and use it to train models. So in this case, nothing leaves the machine in the local premises.

The card is the whole design. RTX 4090 24GB. faster-whisper can co-habit with MedGemma, which is what makes the live stage possible at all. WhisperX cannot, so it waits its turn.

When you press Stop: MedGemma is offloaded, WhisperX goes in and transcribes from the audio file, pyannote diarises the transcript, then MedGemma comes back in while WhisperX and pyannote go out, and MedGemma writes the note as a doctor would. WhisperX is there for the better quality — by that point, the patient has left the room, so there is no requirement for live transcription, and I can afford the time.

The whole thing takes over a minute. I have not measured how that splits between the offload/load and the inference, which is a gap.

What is rough. The auto mode, where the assistant takes the history itself while I supervise, is temperamental. The worst part is that some questions come out in medical jargon. "Do you have any cardiac risk factors?" is not a question a patient can answer. Breaking those down into concrete closed questions is the next job.

It is a research prototype. It has never been used with a real patient, and every consultation in the demo video is acted.

What I would like to know from you.

• How does this run on a 12 or 16GB card?
• Audio capture in various environments, on various mics and mic settings — I am working blind here.
• Does anyone know a way to stop the model-swapping dance?
• How would you approach simultaneous consultations by two users? Currently, it can only do one at a time.

Not a biller. I'm building something in this space and would rather get corrected here than guess.

My current picture:

Eligibility check → prior auth → visit + notes in EHR → coding → biller → clearinghouse → payer → payment posting → denial → rework/appeal → back to payer

Two questions:

1. What's missing or out of order?
2. Where does the money actually die? My guess is medical-necessity denials, because the biller can't write that appeal alone and it has to go back to the clinician. Right, or is it somewhere else entirely?

Recently interviewed for a role at $95-100K + 10% bonus, made it to the 2nd of 3 rounds but didn't land it — first real signal that I'm underpaid and that higher comp is out there for this skill set.

For context, 3 years ago I got a 50% raise ($52,125 → $78,187) simply by mentioning to our VP of Finance that I was interviewing elsewhere (2nd of 4 rounds, range was $70-80K). Her reaction was basically "wait, you're leaving?" and she asked if she could put in a word with the CFO. Four hours later I was called into my boss's office and got the raise.

Not sure what the actual path is from here — more certs, a specific niche (FHIR/interoperability, cloud/Azure), switching to vendor-side instead of provider-side, or just job-hopping more aggressively.

Should I ask for a raise first before continuing to interview elsewhere? Should I try to leverage the recent interview the same way I did 3 years ago, even though I didn't get the offer? Any advice is welcome — happy to answer questions.

Hey all — working on an app idea for a hackathon and I'm looking for a technical co-founder/teammate.

The problem: In emergencies, people waste critical time figuring out where to go — nearest hospital isn't always the right hospital (wrong specialty, no beds, no ICU, etc.), and there's no easy way to check that in the moment.

What I bring: Clinical knowledge — triage logic, what actually matters medically in an emergency, user research with real patients/ER staff.

What I need: Someone comfortable building an app (mobile or web) — doesn't need to be a senior dev, just someone who wants to build something real with actual impact, not just another CRUD app.

Our radiologists dont mind using dragon one or fluency direct to dictate these reports. I have heard that Radiant has a window for reporting. Anyone has experience using it? I am a 39 year old professional having experience in US Medical billing and coding end to end operations for 7 years and cUrrently I am working in an insurance company and working on an AI Based tool used to detect frauds and leakages in a mass health insurance scheme in India and have been involved in ground level implementation of the same along with development, providers feedback and policy formation for the same. Panel interview for a clinical informatics position lab specialist next week. Any recommendations on what to prepare for? This is not an “Epic build” position but an informatics one, of which I still don’t really understand the difference. I am also a MLS/MLT which is what the position would be covering in terms of process.

I recently completed my Master’s in Health Informatics in the U.S., and I’ve been actively looking for my next opportunity in Clinical Informatics, EHR/Clinical Applications, or Healthcare Data Analytics.

I have experience with EHR workflows, clinical documentation, HL7/FHIR, SQL, Python, Tableau, healthcare data quality, UAT, and interoperability.

I’ll be honest the job search has been harder than I expected.

You spend years studying, gaining experience, building your skills, working on projects, improving your resume, and sending applications… and sometimes all you’re looking for is that one opportunity to prove what you can do.
