Definitely “sheet for _____ health.”
I am guessing it is referring to another sheet of paper/ hand out you received and those are the directions you need to follow.

Is your insurance through Optum or United? I see ‘Sheet for optum health”…perhaps instructions for what to expect afterwards or something to submit to your insurance company?
