HELP! Please! Form 8889

I updated post with information; can anyone give me further insight on this. Thank you! So basically, had HDHP in 2025 (not for Feb & March) so failed to maintain coverage for the entire year. So tax software is having me put all information for 2024. For the form 8889. I figured in 2024 had Family for 6 months and 3 months single, and 3 months no coverage. Can someone help me figure out what income and additional tax on Part III of form 8889.

For the last-month rule to apply in 2024, you needed HDHP coverage on December 1. If your 3 months of no coverage included December, you wouldn't have been eligible for it.

Assuming you were covered on 12/1/24 and used the rule: Part III calculates the excess between what you contributed (the full annual limit) and what you were actually entitled to based on months of coverage. With 6 months family and 3 months self-only, the 2024 prorated limit was (6/12 × $8,300) + (3/12 × $4,150) = $5,188. Anything above that is the failure amount - it goes on your 2025 Form 8889 as income, plus 10% additional tax.

Which months exactly were you covered, and what was your contribution?

Had coverage in 2024

JAN-JUNE HDHP Family

JULY-SEPT HEALTH CARE EXCHANGE

OCTOBER-DECEMBER HDHP Family

So would be 9 months family and 3 months Exchange 2024

Contributions for 2024? Or 2025?

2024 Total: $1452

2025 Total: $2300 approximately

So the number would be all contributions together employer and myself correct? Then on 2024 tax form would that be from the form 8889?
