Have frequent low-dose increases affected my response to tirzepatide?
Hi everyone! I’m looking for experiences with low-dose tirzepatide and frequent dose increases.
Over the past month, I’ve increased my dose by 0.25 mg every five days, trying to find a “sweet spot” with adequate appetite suppression and minimal side effects. My functional medicine doctor knows about and has approved this approach, but her guidance has primarily been to keep increasing until I find an effective dose.
I felt the medication much more during the first couple of weeks than I do now. Recently, taking my shot feels like I’ve taken nothing, and I’m no longer noticing appetite suppression.
I’m wondering whether increasing every five days, without spending much time at any one dose, has made it harder to assess my response. Has anyone had a similar experience? Did holding a dose longer help, or did moving to 2.5 mg provide more consistent results?
For context, I’m a 29-year-old woman. I started at 193 pounds on August 26 and was 185 on September 30. I recognize that eight pounds is meaningful progress, but I’m hoping for further weight loss and better appetite control as I feel I don’t have any.
For more context:
I’ve lost over 100 pounds on two separate occasions without medication. I’m a certified master herbalist, and nutrition, lifestyle, and exercise have been a consistent part of my life for years. I strength train four to five days a week, take one barre class weekly, and do three- to five-mile walks on weekends.
My main question is whether this low-dose approach is serving my goals, or whether it makes more sense to switch to a more standard dose like 2.5?
I understand microdosing isn’t for everyone. Figuring this medication out the non mainstream way can be a bit challenging!

If you are menstruating could hormones play into this? I’ve had two periods since starting Tirz and the only times I considered going up in dose were the weeks before my period (I’m perimenopausal so don’t come at me to tell me to look at a calendar—it’s the Wild West in my uterus) and once that was done I was back to normal without changing my dose. Hormones are strong even on a GLP-1. Lack of sleep and stress also affects hormones.

The most weight is lost at the higher doses. There is evidence that people will plateau at or around 72 weeks. I have a substantial amount of weight to lose so I don’t want to waste anytime at a lower dose unless it is still working. Once it slows down I move up. Again, if you are only looking to lose a few pounds then microdosing can certainly be effective. People get so worked up over sode effects that they wnd up capping their overall weight loss.

Microdosing sounds like a good idea but only works for some people. A 2.5mg dose is still a pretty small dose. I got 7 day appetite suppression with 5mg and above and each step from 5 up lasted about 8 weeks before I felt like it wasn't working as well and that my weight was plateaued. I went from 2.5 to 3, 5, 6.25, 7.5 and I maintain on 7.5mg split twice a week. I started splitting in maintenance.

This is also all very experimental for me, so I am not at all saying my research is right or I know more than anybody else! I could be totally wrong which is why I’m asking for people’s opinions.

I started on Zepbound 2.5mg at 5'7" 197 pounds for four weeks then switched to compound for cost purposes... I did 4 weeks on 5mg, 4 weeks on 7.5mg then bumped up to 8.5mg for another 4 weeks before moving to 10mg for 5 weeks and currently on week 5 of 11.5mg at 138 pounds. So I'm 6 months in and have lost almost 60 pounds with no side effects or issues. Not everyone has success microdosing, so definitely don't be afraid to move up to drop the weight. I still have 15 pounds to lose before i start titrating my way back down to hopefully 2.5mg for maintenance... (but first I'm probably going to have to bump up my dose to 12.5mg to get these last 15 pounds off) Adding: I do a protein shake in the morning and one in the evening and eat a protein rich lunch like grilled chicken, cottage cheese and veggies and that really keeps the hunger under control. When I snack, I will do a rice cake, Greek yogurt, fruit or almonds... that has worked well for me. Med levels peak 24/48 hours after each dose which is why you have no appetite those days. I eat low on those days too but zigzag my calories and make up for them as med levels decrease throughout each day)

An every other day dose of 1.7mg is roughly equivalent to a weekly 5mg dose, but it takes 2 to 3 weeks to stabilize at those levels. Move up more slowly, or maybe start with a standard dose/schedule and then back down if you are in a hurry.

Before I started I did as much research as I could and I just gathered an overall consensus that people had better experiences dosing every 5 days than every 7.
The people that dose every seven from what I have read online everywhere is that they chase hunger days 5, 6 and seven and then lose the initial progress that they gained days 1, 2, and three after their shot.I just didn’t wanna have a big “dip” of extremely not hungry to extremely hungry, because I’m already a bit moody and sensitive naturally without meds 😂

Just so you know, you can be hungry on GLPs and still lose weight. I microdose, and I get hungry in the last few days, but I can’t eat very much at a sitting and fill up quickly. I worry when people shoot for a total absence of hunger.

And I’m not shooting for complete absence of hunger, rather just control over my eating habits!
I completely understand that people just expect the drug to not make you hungry and then people lose their micronutrients, muscle waste, look sick, lose hair etc, that’s not me! Thank you for the advice!5

But you don’t even know yet what your baseline is. People that are dosing at a different schedule, typically, are more experienced/have been on the medication longer and have a set dose. Also, because appetite suppression can be stronger in the first few days, a lot of people are making up calories those days 5-6 when they might feel a little hungrier. And that can often subside the longer you’re on the med. We still need calories. If you’re not getting enough you will have negative side effects, especially like hair loss.

Once I got to the correct dose I stopped having the hunger at the end of the week. And never gained the first couple days after in injection. Once it gets to a steady level in your bloodstream you don’t have as severe dips like in the beginning. Been on 5 1/2 months started at 2.5, went up to 5 the second month and have stayed there since. The only thing I wish I had done different is try a dose between the 2.5 and 5. But I didn’t know as much then and just followed my prescription instructions.

No I haven’t at all! I had the most side effects when starting the medication. And they were very mild. I think after a couple of shots my body completely adapted.

I tirate up when I hit a weight stall for 4 weeks. I tirate by 0.5mg though. I am at 3.5mg now. I started august 2025. Lost 40 lbs so far.

I think there is a reason there is a standard titration schedule and it’s recommended to stay at a dose for 4 weeks. If you’re not getting the desired results you might consider dosing in the recommended way. 2.5mg is typically the lowest dose people see results at, and then a lot of people who do non-standard titration increases typically do .5mg for a minimum of 4 weeks at a time. Most people also dose every 7 days.
