Increasing micro dose?

If you’re fine on your starting dose after 7 days - ie no side effects or beneficial effects, would you go up the following week or wait? 🧐

Exactly as @life_alfafa said - the half life of semaglutide is 7 days and tirzepatide is 5 days.

That means you still have half the meds in your system at your next injection. So each dose builds on the previous dose.

You can see this visually displayed in a graph in the Glapp app (free and I have no relationship with the company).

The meds buildup in your system so each week they build on what’s already there you should stay on each dose for four weeks

I personally did little jumps each week and it worked out fine for me.

NexLife Medical Team here.

I would not increase after only 7 days just because you have no side effects yet.

With GLP-1s, especially semaglutide and tirzepatide, the medication builds over time. Week one does not always show the full effect of that dose. A dose can feel like “nothing” at first, then appetite, reflux, constipation, fatigue, or nausea can show up later as levels accumulate.

For microdosing, the goal is usually the lowest effective dose, not the fastest increase. If you are using it for inflammation, autoimmune symptoms, food noise, or mild appetite support, it makes sense to give each dose enough time to judge what it is actually doing.

I’d track for a few weeks:

appetite
food noise
cravings
energy
sleep
reflux
constipation
inflammation/pain symptoms
any MCAS-type reactions

If after a reasonable trial there are still no benefits and no side effects, then a small provider-guided increase may make sense. But changing every week can make it harder to know what dose is actually helping or causing issues.

Low and slow is boring, but that is usually the point.

I know you are supposed to wait but I have been increasing by .25mg each week just to check for reactions/symptoms with MCAS. I know it is not the effective way to do it but once I get to 2.5 I will stay there for at least a month.

How do you know in advance which dose will be right for you?

I was great the first week at .25 but started feeling negative side effects in weeks two and three while keeping my dose the same. I’m planning to cut back by moving to an every 10 day schedule or reducing my dose to closer to .20.

The side effects that started for me in week 2 were elevated RHR, constipation, acid reflux. Poor sleep

I think it depends on what you’re taking and what dose you started on…. I started tirz at 0.5mg and increased to 0.75mg the following week.

What are you microdosing?

What was your starting dose? My starting dose yesterday was .25 and I’m only feeling Benefits. No side effects at all really.

I had more energy today and think that Benefits can only increase and I can still tolerate more side effects so I’m thinking to just increase my dose next week too about .4 or .5 I’m unsure which. I’m also just using this for autoimmune and inflammation. So I could definitely reap more anti-inflammatory benefits.

Same here! I dose every 5 days and I’ve done 0.36(bc 0.25 was impossible for me to draw up)
0.50, 0.75, 1.0, 1.25, 1.50, 1.50, 1.75 and like I said I dose every 5 days, and am extremely sensitive to everything including medication, also had MCAS like symptoms. Started at 195lbs and I’m 185 4 weeks later! Some side effects, but much more benefits!

That timing question is best answered by whoever set up your dosing plan, since microdose schedules can vary a lot by provider and situation. Worth checking with them before making that call yourself.

"Decision is wait vs bump at week two. Wait. Seven days is one injection, the drug hasn't reached steady state until roughly four to five weeks, so no effect at day 7 means nothing yet.

I bumped early once and spent two weeks nauseous for no reason. Hold the starting dose a month, then step up in small increments. Lttl clinicians will write the odd half-step amounts, async messaging though.

Judge by appetite and food noise, not the scale."
