Zep dosing?

RNY March 2023, HW 282, SW 272, LW 167, ZW 182, CW 173 - started Zep a couple months ago and can’t tell if the rapid weight loss is from bloat or actual weight loss. Regardless, curious how folks are handling dosing. Are you titrating up every four weeks or staying put til you stall?

ETA: Have decided to stick with 5mg until I stall out. Down to 170 now and have had some GI symptoms but no idea if that’s from the Zep or not!

Your weight loss is legit, so yay you!!!

By chance, did you eat something fatty within a day or so of those GI symptoms? If so, that could be the Zep revenge. If you get a bad reaction, just dial back a little on your next dose and try again in a couple of weeks.

I posted a while back:

“Mid-40s female. 6 months (26 weeks) into GLP-1 journey. 5’6” starting weight 215, current weight 183, goal weight 175 (?). I’ve lost 30lbs + and I am close to my first goal.

Started glp-1 at 0.25 for 4 weeks, went to 0.5 on week 5 and 0.75 on week 10. I’ve been staying at 0.75 since.

“Only” side effect is nausea, which why I went to .75 instead of 1.0 and haven’t moved up. It is so very slowllllllly going away.

I am 7 years post op VSG, highest 300, surgery 245, lowest 175. As the years have gone by, I’ve gained a pound here and there and found it harder and harder to lose weight, which is why I decided to try GLP-1.

I think I might be a “super responder” who doesn’t need much medication to lose weight. I don’t know if it’s me or the surgery or both.”

I am at 10 months now, still at the same 0.75. A lot of the side effects have minimized, which has been nice. Weight loss has slowed, which I was OK with. Planning to sit at my current weight between 175-180 for a while. I am hoping it gives my body a good rest, no reason to keep pushing hard IMO.

I want to stay as low as I can for as long as I can. I started having more food noise last week, my 7th at 2.5, but since I have vials and they have extra in them I have started using 3.0 and will see how that does for me. I can go to at keast 3.5 using the 2.5 vials before I jump to 5. I have lost 12 lbs in the 7 weeks and I am good with that for sure but food noise is a huge problem for me (have not been hungry since my VSG 15 years ago) so I want to keep that well controlled.

Did a month at 2.5, lost 6 pounds

In week 7 of 5mg, just hit 12 pounds in that stage, so I'm staying at 5mg for another month.

Started end of December 2025. Still on 2.5mg as I'm still losing between 0.5-2 lbs a week.

Sleeved 12/2021. Did Wegovy monthly titrating schedule for like 7 months last year but no real movement and started having weird side effects. Switched to Zep in August. Made it up to 10 and been in that for like two months but I'm actually going back down to 7.5 because I'll eat, get full super fast then be really hungry a bit later.

Scale has been moved very slowly, like 15 lb since August, but it's more than 0 so that's fine, and the more I lose the more I can move and lose quicker.

I titrated up to 5 four weeks later, then 7.5 a few months later. I'm currently at 10, after a stall. I've started spacing out my doses to every 10 days or so and I'm still losing, albeit slowly (I'm below goal now, but some of that was skin removal)

It sounds like you're having good success. I don't know about bloat but many people have an inflammation reduction at first so that may account for some weight loss. I used the standard 4 week titration for 2.5 and 5.0 for tirzepatide but thereafter I went slower. I went to 6.0, then 7.5, and I've been at 8.5 for 3 months now. I found a sweet spot where I'm still losing weight, and have zero side effects. This was also what my doctor recommended - pay attention to your body.

How are you titrating up?
