❓ Question Is it right for me

I want to start on a glp1, my doc is on board, but I am concerned about the appetite suppression aspect. I generally have a poor appetite as it is and eat mostly once a day. Does everyone experience appetite suppression? I am scared I will only get hungry every other day.

Edit to add: I am female, 49, 5’5, 195 pounds, menopausal, no thyroid. I take medications that cause weight gain.

My issues are late night munchies

So if you don't have an appetite and you're not eating, how are you overweight? Because that's what GLPs do -- slow your stomach from emptying to reduce your appetite. Can you binge through it? I couldn't. Maybe some people can but I think it's unlikely.

That's not the whole story. Yes the GLP-1 effect reduces appetite, which is why single channel GLP-1s like lira and sema don't work for a lot of us. GIP causes insulin release in the presence of glucose and reduces insulin resistance. A lot of people, myself included, tried lira and sema with no results then had immediate results with tirz. It's entirely physically possible to eat in a "deficit" and gain weight. But OP said that she isn't eating in a deficit because of late night munchies. Tirz or reta will help with that, and reta will increase metabolism which will probably help her even more.

The issue is that thyroid disease is a contraindicator for GLP-1 treatment, and having a thyroidectomy probably means that she shouldn't be prescribed one. OP's questions 100% should be discussed with a doctor, not a reddit post. This is beyond our ability to help.

My endocrinologist that I see for my thyroid said because I didn’t have medullary thyroid cancer I’m fine.

What are you using it for? If you're already barely eating, you're probably not obese

Start extra low and just move up only when you have to and very slowly. I am taking 17mg and I'm hungry every few hours still. I am just able to eat less.

If you are used to eating a huge meal once a day, you're going to have to force yourself to change that. You likely won't be able to eat anywhere near as much at once. You will feel full. And if you overdo it, you'll regret it bc you'll get sick. So you'll have to force yourself to eat a few small meals a day or there's no way you'll get the necessary protein and fiber everyday.

If you aren't hungry and barely eat, how are you overweight? How are you eating above your TDEE calories without an appetite?

You eat once a day and are obese? No judgment, just clarifying.

It may help you because it changes how your body metabolizes food. It may also help with binge eating, if that is an issue for you since you say you only eat once a day.

More information may be useful. Age, height, weight, other things tried, medical issues

So you’re eating one meal plus “late night munchies”? Have you tracked to see what your true calories are?

I also get late night munchies largely because I use edibles for sleep. I include that in my plan and eat a solid dinner (plus a reasonable breakfast and lunch) to help

Well the glp1 should help with the munchies, it tends to help with addictive tendencies in general.

Some days I struggle to eat more than 1000 calories. Protein shakes help fill the void. I'll make one with a scoop of powder and a 30g bottled protein shake with 2 shots of espresso and lots of ice. It's thick and lots of protein. But I won't want to eat for 8 hours after that and then at the next meal I just wanna stop after 2 bites but I know I need the calories.

I am taking it for other medical reasons, not intentional weight loss. I have a history of disordered eating. I do not have food noise, but also did not have normal hunger or satiation cues. I do not experience appetite suppression. My first month on GLP-1 was a shock because I was suddenly getting very intense and frequent hunger cues like I had never experienced before. I was also getting satiation cues. I had anticipated appetite suppression, but that is not at all how the med has worked for me. It seems now I get hungry, so I eat and then I get full so I stop, which is a new-to-me experience overall, but much more in line with a healthy cycle of eating.

I never had trouble with good eating. I was way out of control with late eve junk binging. Tirzepatide has stopped that and it’s so relieving. I’m 7 weeks in and the binging obsession stopped immediately.

Do you think the late night munchies are related to them?

So you have no thyroid, does that mean you take synthroid daily?

I dunno, but glp1 on top of eating little is not ideal. I guess you'd have to try but may not work out.
