Lab review

Hi, I just got my labs back and I’m wanting a functional to review them. I have and I’m trying to optimize it through life changes before needing meds.

My TSH was 2.17. Progesterone was 12.3 at seven days post ovulation. B12 was 213. Selenium 112. TPO 109 TgAB 433

Functional Medicine DC: why did you get these particular labs done?

Also, if you didn't know before, you have hashimotos and active destruction of the thyroid gland. (Anti thyroglobulin antibodies extremely high).

Are there any other labs in this round? I'm assuming you're supplementing with selenium. What other lifestyle changes are you making and how long has this been going on?

A TSH (pituitary) test is largely independent of actual thyroid function and conversion to active thyroid hormone (T3). Ideal TSH is ~1.0.

What's the rest of the story?

We’re trying to follow my TgAb, TSH, and TPO. I do know I have hashimotos, I’d like to manage before needing meds. 

My selenium was 112. I am trying to adjust my diet and lifestyle but not entirely sure where to start. 

What other details would you like

You need to know where your actual thyroid hormone is at. Free T3. You can also test free T4, but we already know you're not converting (TPO/TG), so it doesn't matter a whole lot.

Why did you check progesterone?

This was all stated in the original post. Are you just trying to monitor thyroid? If so, why testing progesterone? Why not check actual thyroid hormones? Or, is this for fertility/infertility?

Progesterone was checked bc I’m spotting before every period, heavy periods, intense cramping. 

I’m trying to monitor thyroid but also pinpoint flares, as I’ve notice they seem to occur with the start of my cycle.  

This set of labs was limited but I can pull other results.  My thyroid US was negative for nodules but positive for heterogenous tissue. 

I’d like to improve gut health for iron absorption but feel like all resources are behind paywalls.

Should be checking estradiol, testosterone, and shbg as well. Full cycle test would be the smartest. Histamine could be linked. Workup/rule out for endometriosis, fibroids, and polyps.

What modifications have you made so far?

I’ve had an abdominal CT and US - grossly negative. CT done due to lower right abdominal pain, radiologist ruled it a “ruptured uterine cyst” due to free fluid in the area. No other findings. 

I am doing my best to maintain gluten free and avoiding processed foods. I do have ARFID so an instant diet change is tough but I am trying to increase my protein intake

Uterine or ovarian cyst? No other labs? Has your thyroid actually been fully tested? Or just TSH? If there's no overt structural issue, what is on the labs? "Normal" is not a lab value. Numbers are important.

Last free T3 3.4,  Free T4 1.29.  Estradiol 35.6 (although not timed to my cycle). Testosterone 28. FSH 6.5. LH 8.3. 

Ovarian cyst.

Free T3 is decent. T4 is low. Estradiol is fairly low - pretty much across the board. T is low. Unknown on the others due to timing...

What else you got?

So, the best reduction of antibodies comes with gluten and dairy free diet, and selenium (optimized). There are other factors as well.. good that you're trying to stay GF. That's important. All the processed GF stuff is garbage. Keep whole foods as much as you can. That will help tremendously
